This risk register template can be used in conjunction with a risk management plan or by itself.  The template is in Excel format but it customizeable according to your needs.  Tips for using the template are included in the article, What is a Risk Register?

Thanks you have saved me a lot of valuable time very well laid out. Im doing OHS diploma i just collecting different formats of risk registers,But I will be using thisas a register in future
